The Illinois Supreme Court Commission on Access to Justice (ATJ Commission) is requesting public comment on six Illinois Supreme Court form sets.
The following draft forms are available to review on the Illinois Supreme Court website:
- Petition for Leave to Appeal
- Answer to Petition for Leave to Appeal
- Notice of Election
- Appellant’s Brief
- Appellee’s Brief
- Appellant’s Reply Brief
Each form set includes a “Getting Started” and a “How To” document to guide court users through completing the forms.
All form sets are available for public comment through Friday, March 22, 2024.
You may submit comments through the Illinois Supreme Court website or directly to forms@illinoiscourts.gov. The Appellate Forms Subcommittee will review public feedback before the forms are formally approved and published on the Illinois Supreme Court website.
Redesigning Illinois standardized court forms
In accordance with its 2023-2028 Strategic Plan, the ATJ Commission is prioritizing updating standardized forms to promote and enhance equal access to justice in Illinois.
The updates include automating forms and translating them into languages relevant to Illinois court users, as well as streamlining processes to ensure forms are easy to file online and in person.
The ATJ Commission released the first court forms in its redesigned format in early 2024 and recently announced that the Child Support Order and Financial Affidavit Suite were republished with minor updates based on its annual review.
