The Illinois Supreme Court Commission on Access to Justice requests public comment on a new Complaint or Petition form set. The public comment period lasts until Thursday, April 3, 2025.
Public feedback can be provided on the Illinois Courts’ website or emailed to forms@illinoiscourts.gov.
The form titles include:
- How to File a Complaint or Petition
- Complaint or Petition
- Additional Defendant/Respondent Information
- Additional Reasons for Lawsuit
Feedback to improve the forms will be reviewed and may help inform any necessary revisions, which will be made before the forms are formally approved and published on the Illinois Courts’ website.
The standardization of statewide court forms for use by self-represented litigants is an integral part of the Commission on Access to Justice’s efforts to promote and enhance equal access to justice in Illinois.
A list of approved statewide standardized court forms can be found on the Illinois Courts’ website. All Illinois courts must accept these forms.
